Our Vision is firmly grounded in the principles of the government’s ‘Living Well with Dementia: A National Dementia Strategy’. Everything we do is shaped by its core aims: early support, personalised and meaningful care, social connection and helping people maintain a fulfilling quality of life.
We understand that every person is unique and has individual requirements. We will take time to ensure that their needs are met and that any preferences and familiar routines are incorporated into a tailored care plan for each person independently.
We aim to give reassurance with holistic and compassionate care, helping to calm any anxiety, confusion or distress. Our approach is to see the whole person not just their illness, by addressing their physical, mental, emotional and social needs to achieve overall wellbeing.
Respite that makes a difference
At Primrose Care Farm, we understand that going somewhere new can feel like a big step, especially for someone living with dementia. Our method is gentle, supportive and centred around each individual.
During the complimentary settling-in day, we spend time getting to know our new companion and their support network. This allows us to understand their needs, interests, personal preferences, what matters most to them and any relevant medical information.
To support this transition, we kindly ask that a member of our companions support network remains available during the initial settling-in period. This ensures the companion has the reassurance they need and helps us build a strong foundation of trust and security from the very beginning.
Each companion is supported by a dedicated key person - someone who takes the time to get to know them well and supports them throughout their time with us. Families and carers also have access to our policies, guidance and information so that everyone feels confident and reassured about the care we provide. Regular reviews with our companion’s support network will ensure we continue to meet their needs as they progress in their journey.
